About this policy
Eye D Vision is the practice brand used by Eye D Vision LLC (Bensonhurst), Eye D Park Slope LLC (Park Slope), and Eye D Vision Bayside, LLC (Bayside). This policy explains how these businesses handle information collected through orthokeratologynewyork.com, and inquiries made through the website. It is separate from our HIPAA Notice of Privacy Practices, which describes the use and disclosure of protected health information in providing care. Where information is protected by HIPAA or other health-privacy law, those requirements apply.
Information collected
- Information you provide: your name, contact details, and the information you choose to send when contacting us about appointments, products, employment, or partnerships.
- Website information: IP address, browser and device information, pages visited, referral sources, timestamps, and cookie or similar identifiers may be processed by the services described below.
- Appointment information: selecting “Book a consult” takes you to our external scheduling provider. Information entered there is handled through that provider and applicable health-privacy arrangements, not a general website contact form.
- Call and text information: contacting an office provides your telephone number and the content of your communication. Call attribution can also associate a call with a website visit or advertising source.
How information is used
Information is used to respond to requests, support scheduling and patient service, handle employment and partnership inquiries, operate and secure the website, troubleshoot problems, understand how people find us, and measure advertising where enabled. Health information is handled under our HIPAA notice and applicable law.
Website services and tracking
- Cloudflare: hosts and delivers this website and supports performance, security and technical logging. Network and device information is processed when you access the site.
- Google Analytics and Google Ads: the main website loads Google tags to measure page visits, referral sources and consultation-link clicks and support advertising measurement. These services may process IP addresses, device and browser information, page URLs and cookie identifiers.
- Ortho-K Candidate Estimator: your age, prescription and health answers are processed in your browser to generate your result; the estimator does not save or send those answers to our analytics. We measure estimator opens, starts, results viewed and consultation-link clicks, including whether the estimator is embedded on the Eye D Vision website. These usage events do not include your answers or candidacy result. The estimator uses Google Analytics with advertising personalization and Google signals disabled in its configuration.
- CallRail: uses cookies and dynamically displayed telephone numbers to associate calls with referral and advertising sources. Existing account connections may report call conversions to Google Ads. We do not record or transcribe calls. Call attribution does not mean a call is recorded.
- Accessibility widget: the All in One Accessibility service provides accessibility controls. Loading its resources involves communicating with that provider.
- Other external services: YouTube video previews, maps, scheduling, financing and other external links may load content from or take you to another provider. Its privacy practices apply to information it independently collects. The separate Eye D Vision website has its own privacy policy and tracking configuration.
This website does not currently install a Meta Pixel. A privacy policy is not authorization to disclose protected health information where HIPAA requires separate authorization.
Cookies and your choices
The site's Google and call-attribution tags currently load without an on-site cookie preference panel. You can manage or block cookies and tracking through your browser settings or browser privacy tools. These choices apply to the browser and device where you make them. Blocking tracking does not prevent you from calling an office or seeking care. Necessary hosting and security services still process requests to deliver the website.
Changing browser preferences cannot undo information already sent to a provider. For questions about website information or your available choices, contact the practice using the details below.
When information is shared
Information may be processed by service providers that support the functions above, by recipients you ask us to contact, and where disclosure is legally required or permitted to protect rights and security. Advertising-related website sharing is described above rather than characterized as anonymous or covered by a blanket “we never share” promise. Disclosures of protected health information must satisfy applicable health-privacy requirements.
Retention and security
Different types of information have different retention requirements. Medical-record retention is governed by applicable law and practice procedures, while website and vendor logs depend on the services and their configured retention settings. Contact us for questions about a particular record or request. We do not promise that all information can be deleted immediately; legal obligations may require retention. No website, email system, or transmission method can guarantee absolute security.
Communicating with us
Please do not send diagnoses, insurance identifiers, payment details, or other sensitive information through ordinary email, social-media messages, or website privacy inquiries. Call your office for an appropriate way to transmit records. Ordinary email and SMS may not be encrypted end to end. Do not use these channels for emergencies.
Children and caregivers
Our website is intended for adults, including parents and guardians arranging care for children. It is not intended for children under 13 to submit information independently. Contact us if you believe a child has submitted personal information without appropriate permission. Patient-record access for parents and guardians is subject to applicable law.
Requests and questions
Contact us to ask about information collected through the website or to request access, correction, deletion, or choices available under applicable law. We may need to verify your identity and authority before responding. Rights and exceptions depend on the information and the laws that apply.
